Military service can bring stress not just for service members, but also for their families, friends, and neighbors. Deployment, reintegration, and combat injuries common today can add to the mental and emotional strain. When those stresses do occur, it’s not always easy for those affected to know where to turn for help.

Health Services, Inc. is committed to meeting the ongoing needs of veterans of military service, active duty military personnel, and their families. Licensed behavioral health professionals undergone specialized training to understand and sensitively deal with military issues and the unique needs of this population.

Veterans Choice Provider

If you are a veteran in need of medical care, including mental or behavioral healthcare, and live 40 minutes away from the nearest VA Medical Facility – or if the nearest VA Medical Facility has a wait time of over 30 days for an appointment – you are eligible to receive medical care from a VA Choice Provider.

Simply dial 1-855-722-2838, press 1 and request an appointment within a Health Services, Inc.’s healthcare facility nearest you.

Health Insurance Enrollment

Health Services Outreach and Enrollment offers assistance to the under and uninsured patients by assisting with help receiving affordable healthcare coverage through the Marketplace or healthcare.gov, which are both based upon eligibility. If patients are not eligible for insurance through the Marketplace, they will be referred to one of our ten locations at Health Services for further assistance.

In addition to assisting with health insurance coverage, we also refer our patients to outside community resources for additional needs, educate them about the Affordable Care Act and how it works as well as educate them on how to properly use their insurance should they qualify. We also assist elderly clients with Medicare questions and we assist consumers with Medicaid via Marketplace.

WIC Enrollment

The Supplemental Nutrition Program for Women, Infants, and Children, better known as the WIC Program, serves to safeguard the health of low-income pregnant, postpartum, and breast- feeding women, infants and children up to age 5.

For years HSI has been honored to be a steward of these Federal funds in our area and provide participants with supplemental nutritious foods, nutrition education and counseling.

Important Information

Anyone interested in applying or re-certifying for the WIC program needs to first call to make an appointment. The

The following items are required at the appointment:

  1. Person applying for WIC (You, Infant, Child(ren))

  2. Proof of identification for parent/caretaker AND person applying for WIC (a driver’s license, birth certificate, hospital birth card)

  3. Proof of residence (a document with your street address on it, such as a lease or bill)

  4. Proof of enrollment in Medicaid, SNAP or Family Assistance or Proof of current household income. Proof of income must include documentation of income for everyone that is working in the household during the past 30 days i.e. weekly pay (4 paycheck stubs), bi-weekly (2 paycheck stubs), monthly (1 paycheck stub).

The documents are reviewed, then the individual is evaluated for a nutritional risk, which is a qualifier for determining eligibility in the WIC program.
